All providers at the Alaska Eye Surgery and Laser Center, Inc. are independent contractors.  Your health insurance is a contract between you and your health insurance.  It is your responsibility to understand your health insurance contract and whether or not your providers and this facility are considered "in network" or "out-of-network" by your health insurance.  We cannot verify or guarantee that your surgeon or anesthesia provider is in-network or a preferred provider for your health insurance.  We are unable to schedule your surgery on the basis of whether an anesthesia provider is "in-network" or "preferred" by your health insurance.  By choosing to have your surgery at Alaska Eye Surgery and Laser Center, you agree that we cannot schedule each surgical case according to each patient's variations in coverage and accepting the financial responsibility of paying each independent contractor for the amount your insurance determines you owe.
